1. Create a 10-item questionnaire and three demographic varieables that address the two research questions. After each questionnaire item, indicate in PRENTHESIS which research question is addressed. NOTE: The demographic varieables should be connected to a strategy related to how you might sub group your data for reporting. 2. Prepare the survey as if it would be distributed for implementation (instructions, demographic questions that would be appropriate and/needed for sub grouping data, including comments, etc). 3. Discuss your scaling choice. (PROVIDE SUPPORTING CITATIONS FOR YOUR CHOICE) 4. Prepare an interview guide that supplements the questionnaire. The interview guide should add depth to your data analysis process. 5. Discuss your approach to ensure these instruments (questionnaire AND interview guide) are reliable and valid. This section should be supported by the literature. NOTE: Reliability is confirmed by the following question: Does the measurement yield consistent, stable, and uniform result? Validity is confirmed by the following question: Does the instrument measure what it purports to measure? 6. Outline a strategy to address non-respondent in the real setting.
