This is the summative theory assessment for the module, and will reflect the % grade for the module (note: both components of placement 2 clinical practice must also be passed.) Guidelines: Your Case Study must be on a patient / client you are actively participating in care. It should accurately reflect the care provision and contain the following elements: • a careful, clearly anonymised review of your patient / client which maintains anonymity and confidentiality of personal details and anonymity of Trust / Placement Area / all personnel involved (breach of confidentiality will result in referral); • appraisal of the nursing / health model used in practice for your patient / client; • socially or culturally specific components of care which demonstrates social / cultural awareness of the patient’s beliefs and needs; • clear aspects of health promotion + patient education activities; • inter- / multi- disciplinary team approach to care provision. It must NOT be a diarised, day-to-day description of care given or an essay about a disease. Presentation: Essay style, use of sub-headings is permitted. It should be word-processed, Arial / size 11 / fully justified / double line-spaced text. Word Count must be included (excluding end-text reference list) and Page Numbers. It is advised that you utilise page headers / footers with only your name/date identified.
