This is for a graduate nursing university in the United States for a thesis project. The prevelance of hypertension (HTN) is high among blacks/African Americans verses whites/ caucasians. I want to point out the main differences specifically Salt sensitivity, pathophysiology, genetics and environment has been shown to account for some of this variations. There is also the question of adherence to treatment, socio economic, and health care disparities. I want to conclude that for blacks there is evidence supporting aggressive treatment with a thiazide type diuretic with either calcium channel blocker or an ACE if there is diabetes present. Additonally the DASH diet has been shown to reduce BP as well. I want to begin the paper with the epidemiology of the disease which I have done. You may give a brief descriptiion of it as well if you like I need help with patho, salt sensitivity, genetics and enviroment to show the differences between whites and blacks which might account to the difficulty and high incidence and prevalence ot HTN. Analysis of the evidence in major studies done in the US like the ALLHAT, AASK AND AAASK studies. Results, conclusion and discussion. This is my main purpose or thesis: You may re write it but let the main points come out with out changing what I am trying to accomplish T he purpose of this study was to determine the differences of hypertension treatment among blacks either using a mono therapy or combination therapy in addition to modifiable risk factors. Most importantly the genetic, environmental and sensitivity to salt particularly in blacks will be emphasize. In addition the various combination therapies will be explore to determine if aggressive therapy is needed to overcome the end organ damage of the disease. The paper will attempt to speculate that for blacks, aggressive and combination therapy should be the first line of treatment in addition to modifying risk factors as the first line treatment. Additionally the paper will analyze the major studies Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial (ALLHAT), African American Study of Kidney Disease and Hypertension (AASK), African American Antiplatelet Stroke Prevention Study (AAASPS) related to hypertension in U.S blacks in.
