Creating a Questionnaire This week practice creating a questionnaire. In the discussions in unit 3 and for this unit, we talked about sampling and ethical issues related to completing a satisfaction survey of a sample of women who received services from any of the 10 shelters in your city. Now it is time to write the survey! You are the new director (congratulations!) of the city agency that provides funding and oversight of all of the shelters. You have decided that the first thing you need is information as to whether the community is satisfied with the program. The “community” has many groups: judges, probation/parole officers and other CJ personnel, staff at the program, the general community surrounding the program, and of course the participants. Choose one of these groups (JUDGES) and create an appropriate survey to gauge their satisfaction with the program. For your paper you should include the following: 1.A cover letter to the potential participants. Explain the purpose of the questionnaire in the form of a cover letter to potential participants. Be sure to include why it is important for them to participate and encouragement to do so. 2.Create an appropriate satisfaction questionnaire for the group you have decided to survey about the services at the shelter. You must include 10-15 questions of varying types, including at a minimum: oTwo fixed alternative questions that have only two choices for the answer, oFive fixed alternative questions with 4-5 potential answers, oOne open-ended question, and oAny other combination of questions to make a minimum of 10 questions. 3.Indicate how you would pre-test the questionnaire, including your test group. 4.Indicate the format you will use to collect the information (mail, internet, phone, group, or face to face interview). Justify your choice. Your paper should be 2-3 pages in length
